1.Finance Business
The Finance Department business is submitted to request approval and ratification by the Board for various warrants and disbursements in order to meet the requirements of Ch. 136.06 (1), Fl Statute. Acceptance and disposal of various Board property and various documents are submitted for the Board's information. At times, approval is requested for various finance related issues.

3.Approval of report and recommendation regarding civil action against Clay County (Rose Acceptance, Inc. v. Jeffrey L. Braswell, et al., Clay County Circuit Court Case No. 17-CA-1089)
The County was served on November 29, 2017, as a defendant in the above-referenced case in which Rose Acceptance, Inc. seeks to foreclose a mortgage dated March 3, 2006, which mortgaged certain real property located in Clay County, Florida. No interest of the County exists as explained in the attached memorandum.
4.Updated Purchasing Agents List Changes
Approval of updated Purchasing Agents list   This list is to be updated when changes are made thereto. Updates have been made for the following Departments: Clay County Sheriff's Office, Extension Services, and Economic & Development Services.
OLD BUSINESS
5.Express Approval of Advance Payment Requirement under Clay County Agreement/Contract # 2017/2018-27 with Municipal Code Corporation
On November 14, 2017, the Board approved entry into a fee agreement with Municipal Code Corporation for legal review of the County’s Code of Ordinances and Land Development Code. The Board approved this agreement through the Consent Agenda, as the agreement was presented to and recommended by the Finance & Audit Committee on November 7, 2017.  This matter is being brought back to the Board for its express approval of advance payment under the agreement. See attached memorandum for further information
6.Interlocal Agreement between the Board and the HFA in connection with the Cassie Gardens affordable elderly housing project - presentation of revised Assignment of Mortgage
As you will recall, the Board approved on August 8, 2017, an Interlocal Agreement between the Board and the Housing Finance Authority (HFA) obligating the County to make a contribution of $80,000 to the construction of the Cassie Gardens affordable elderly housing project. The Board also approved the execution and delivery of an Assignment of Mortgage upon closing. The Board previously signed the Assignment of Mortgage but it now needs to be revised. See attached memorandum for further details.
7.Library Plan Review
Presentation of the library plan which includes increasing hours, restructuring staff and changes to the organizational chart.
PLANNING AND ZONING
8.Public Hearing to Consider Zoning Application Z-17-10, BA to RA, Foxwood Drive
Application Z-17-10 considers rezoning from tax parcels  07-04-26-012920-000-00,07-04-26-012921-000-00, 07-04-26-012942-000-00,  07-04-26-012943-000-00  from BA to RA located in Foxwood Unit 1.  The subject parcels are located in Commission District 3, Ms. Hutchings and in the Orange Park Planning District. (Planning Commission voted 7-0 to approve)
9.Final Public Hearing to Consider Rezoning Application Z-17-12 BA-1 to BB-4 263 College Drive (PC vote 4-0 to approve)
10.Public Hearing to Determine Whether the Proposed Changes to the Governors Park DRI area is a Substantial Deviation and to Consider Proposed Changes to the Development Order
The Board will determine whether the proposed changes to the Governors Park DRI are a substantial deviation and, if not, will consider the proposed changes to the Development Order.
11.Public Hearing to Consider Zoning Application PUD 17-04, AR to PUD, Washington Avenue Residential
Rezone from AR to PUD on 35.08 acres, located on north side of Washington Avenue from Spring Avenue to Turkey Avenue.    The proposed PUD includes 125 residential lots, 5,500 square feet in size and 4.55 acres of open space.  Access is proposed on Washington Street a little east of its intersection with Parkridge Avenue.  (Planning Commission voted 6-1 to deny)

12.Lien Reduction and/or Partial Release

Case No. CE-15-058; 179 Arthur Moore Drive, Green Cove Springs, Florida.  Robert James Ballew and Kathleen M. Ballew, previous property owners.  Bryan D. Benson, new property owner

13.Agreement-Two Fold Water Engineering, Inc.
Approval for both the waiver of the Purchasing Policy as it may apply and the Agreement with Two Fold Water Engineering, Inc., for the inspection-maintenance and monitoring of County owned water treatment facilities, for the term of January 1, 2018 through September 30, 2020, at the total cost of $1,590.00 per month for all sites.  Funding Source: Various Departments (see Agreement/Contract Review Form sheet)
14.FY17/18 Funding Agreement-Clay County Behavioral Health Center, Inc.
Approval of the FY 2017/2018 Funding Agreement with the Clay County Behavioral Health Center, Inc. (CCBHC) for mental health, alcoholism and drug abuse services, for a term of one year, in the amount of $350,000.00.  Funding Source: 105-2604-531400 (County Alcohol & Other Drug Abuse Fund / Drug Abuse & Education / Medical / Dental / Related Support) (C. 15.Amendment to the Capital Improvement Elements of the Capital Improvement Program (CIP)
Approval of ordinance amending the Capital Improvement Elements of the Board's Capital Improvement Program and approval of a resolution amending the Non-Capital Improvement Elements of the Capital Improvement Program. Funding Source: Various Capital Improvement Projects Fund accounts (C.Meng)
16.Bid #17/18-1, Construction of Fire Station 11

Approval to post notice of intent and award Bid #17/18-1, Construction of Fire Station 11 to STG Contracting Group, Inc. including Alternate No. 1 (Vehicle Exhaust System) for a total cost of $2,453,919.18. Approval of the bid will be effective after 72 hour bid protest period has expired assuming no protests are received.   Also request approval of the Construction Contract with STG Contracting Group, Inc., contingent upon budget transfer to appropriate necessary funds. Funding Source: 305-6055-562000 (Capital Improvement Project Fund / Station 11 Replacement / Buildings) (L. 17.RFP #17/18-4, 911 Maintenance Services
Approval to post notice of intent and award RFP #17/18-4, 911 Maintenance Services to the only bidder AK Associates at a cost of $513,000.00.  Approval will be effective after 72 hour bid protest period has expired assuming no protest are received.  Funding Source: 133-2108-531000 (911-Wireless Fund / 911-Wireless / Professional Services) (T. 18.Kraus Associates, Inc., d/b/a AK Associates-911 Maintenance Services
Approval (following award of RFP #17/18-4 and expiration of the 72 hour protest period) of the Agreement with Kraus Associates, Inc., d/b/a AK Associates, for 911 Maintenance Services, in the total amount of $513,000.00 for a three year term. Funding Source: 133-2108-546100 (General Fund / MIS / 911 Repairs and Maintenance)
19.Approval of Fifth Amendment to 800 MHZ Public Safety Radio System Agreement
Request approval to waive the Purchasing Policy as it may apply and approval of the  Fifth Amendment to 800 MHZ Public Safety Radio System Agreement with Motorola. This Amendment includes revisions to the Statement of Work, Equipment List, Contract Design Review Document and an extension of the Final System Acceptance Date with no change to the contract amount.
