		5.	C&H Marine Construction, Inc. Agreement for Camp Chowenwaw Jungle Trail Boardwalk and Fishing Pier Replacement
			Approval of the Agreement with C&H Marine Construction, Inc. for the Camp Chowenwaw Jungle Trail Boardwalk and Fishing Pier Replacement Project, for a term of 120 calendar days, at the cost of $274,300.00. 		6.	Jacksonville Area Legal Aid, Inc. FY18/19 Funding Agreement
			Approval of the Fiscal Year 2018/2019 Funding Agreement with the Jacksonville Area Legal Aid, Inc., for the term of 10/1/18 - 9/30/19, in the amount of $149,250.00. Funding Source: 115-2701-534000 (Legal Aid Fees Fund / Other Contractual Services)
		7.	911 DATAMASTER Software Database Agreement
			Approval of advance payment, sole source designation and the Second Renewal to the 911 DATAMASTER, Inc. Agreement No. 14/15-70, for 911 software database support services, for the term of 1/1/19 through 12/31/19, in the amount of $7,049.00. 		8.	State of Florida Generator Memorandum of Agreement
			Approval of the Memorandum of Agreement (MOA) with the Florida Division of Emergency Management, regarding the distribution of generators to Clay County, for a term of 19 years, at no cost to the County. Funding Source: Not Applicable
		9.	KBT Contracting Corporation Agreement-Thrasher-Horne Door Retrofit
			Approval of the agreement with KBT Contracting Corporation for the Thrasher-Horne Conference Center Door Retrofit Project, with the term extending through May 1, 2019, at the cost of $94,700.00. 		10.	Physio Control, Inc. LifePak Maintenance Agreement
			Approval of the Agreement with Physio Control, Inc. sole source and advance payment, for the maintenance of the LifePak cardiac monitors, for the term of 10-1-18 through 9-30-19, at the cost of $60,357.68. Funding Source: 001-2101-546100 and 142-1703-546100 (Gen. Fund / Rescue-Repair & Maintenance / Fire MSTU / Fire Control-Repair & Maintenance)
		11.	AT&T Agreement Amendment-Add On #11
			Approval of the PRI and Megalink Agreement with AT&T for a period of sixty months, as needed, at the contractual established rates. Funding Source: 001-0107-541100 (General Fund / MIS / Telecommunications)

			The Finance Department business is submitted to request approval and ratification by the Board for various warrants and disbursements in order to meet the requirements of Ch. 136.06 (1), Fl Statute. Acceptance and disposal of various Board property and various documents are submitted for the Board's information. At times, approval is requested for various finance related issues.

		14.	Agreement between Clay County and Urban Jacksonville, Inc. d/b/a Aging True Community Senior Services & Divisional Transfer
			Agreement between Clay County and Urban Jacksonville, Inc. d/b/a Aging True Community Senior Services to provide funding for services rendered in connection with the Older Americans Act; and divisional transfer to support it.
		15.	Agreement between Clay County and Jacksonville Transportation Authority & Divisional Transfer
			Agreement between Clay County and Jacksonville Transportation Authority to provide funding for services rendered in connection with the Transportation Disadvantaged Program; and the divisional transfer to support it.
		16.	Agreement between Clay County and the Clay County Council on Aging & Contingency Transfer
			Agreement between Clay County and the Clay County Council on Aging to provide funding for transitional expenses; and the contingency transfer to support it.
		17.	1st Amendment & Modification to Commodities Distribution Agreement with Challenge Enterprises of North Florida, Inc. & Funding to Support
			Approval of the First Amendment and Modification to Agreement No. 2018/2019-7 with Challenge Enterprises of North Florida, Inc., for additional services and compensation regarding the full management of the Commodities Distribution Program, for an additional cost of $18,252.75 during the period of January 1, 2019 through September 30, 2019. This Amendment also allows for in-kind motor vehicle maintenance services in the amount of $20,000.00 which is in connection with the transportation services that will be provided by Challenge Enterprises for certain individuals with disabilities. Additionally, approval is requested for the divisional and contingency transfers to support it.

			The Department of Tourism is confirming the use of the County's Logo and requesting a waiver of the Purchasing Policy for the approval of Agreement with Strive Media Innovations, LLC (d/b/a Trips to Discover), for marketing advertisement, to be placed on 4/1/19 and conclude on 3/31/20, at the cost of $15,000.00. 		23.	Animal Care Expo 2019 Application

			Clay Electric Cooperative requests the Board's approval to use 12 voting machines and related tabulation equipment for the Cooperative's 81st Annual Meeting on March 28, 2019 and approval for the Supervisor of Elections Office to process absentee votes for the 2019 elections.
