1) Approval of Supplemental Agreement No. 3 with Florida Department of Transportation related to the Local Agency Program (LAP) Agreement for CR 209 Safety Improvements Project.  This Supplemental allows for a time extension which is necessary to finalize end of contract negotiations and closeout documents. This amendment will extend the end date from June 30, 2022 to December 31, 2022.  Additional approval of Authorizing Resolution is requested. 

2) Approval of Supplemental Agreement No. 4 with Florida Department of Transportation related to the LAP Agreement for CR 209 Safety Improvements Project.  This Supplemental allows additional funding in the amount of $4,607.70.  These additional funds are necessary to pay the Contractor for additional material needed to complete the project. Additional approval of Authorizing Resolution is requested along with approval of accompanying Budget Resolution. 

The Safety Improvement project has been completed. Amendments are part of final  closeout process. 

Regarding Amendment No. 3: Original Agreement is due to expire on June 30.  Additional time is needed to close out the project with FDOT. The Amendment provides for aa time extension to December 31, 2022.  

Regarding Amendment No. 4: Material increase was due to a miscalculation during the design phase of the project. The Contractor has completed the project by including the additional required materials. 

FDOT required two separate Amendments for the extension and additional funding.
