Approval of the Lease Agreement with Urban Jacksonville, Inc., d/b/a Aging True Community Senior Services, for the lease of the following properties, for the term of 10/1/2023 through 9/30/2024, with the lessor paying $1.00 per year as rent.

B) 414 Stowe Avenue, Orange Park, FL 32073

C) 3916 Section Street, Middleburg, FL 32068

D) 125 Commercial Circle, Keystone Heights, FL 32656

Lease Agreement in conjunction with the FY23/24 Funding Agreement with Urban Jacksonville, Inc., d/b/a Aging True Community Senior Services.  Agreement provides for services or use of the kitchen at 604 Walnut Street during an emergency.  Agreement also provides for the use of the premises, buildings, and/or any portions of the buildings by the County with other persons, organizations, and entities on times and dates when not in use by the Lessee.
