VIDEO1.Acceptance of Federal Funding Assistance Regarding the Edward Byrne Memorial Justice Assistance Grant (JAG) Program (D. Garrett/R. Vanzant)

Acceptance of Federal Funding Assistance Subgrant #2021-JAGC-CLAY-1-3B-010, Drug Reduction and Enforcement Initiative, with the Florida Department of Law Enforcement (FDLE), with Clay County being allocated $48,611.00. Funding Source: Grant revenue to be included in the FY 21/22 Budget.

VIDEO2.Modification #1 to Agreement #2019/2020-90 with the Florida Division of Emergency Management-Sheriff's Orange Park Substation Wind Retrofit Project and Budget Resolution Approval (A. Dangerfield/R. Vanzant)

(1) Approval of Modification #1 to Agreement #2019/2020-90 with the Florida Division of Emergency Management, regarding the Orange Park Sheriff's Substation Wind Retrofit Project (FDEM #H0218, 4337-126-R), reinstating the agreement, extending the agreement to 2/28/2022 and increasing the Federal Share of the agreement amount by $23,551.25.

 

(2) Additional approval of the accompanying Budget Resolution to record the unanticipated Federal Share revenue is also requested. Funding Source: 140-140-331505

VIDEO3.RFQ #20/21-43, Professional Design Services for Animal Services Shelter and Care Facility (K. Smith/T. Nagle)

Approval to accept staff’s ranking and select the number one ranked firm for RFQ #20/21-43, Professional Design Services for Animal Services Shelter and Care Facility and authorize staff to begin negotiations with the top ranked firm. Approval will be effective after the 72 hour period for protest has expired.

Authorization to reject proposals received for RFQ No. 20/21-40 Design Services for the Northeast Florida Multipurpose Youth Sports Complex.  These services are no longer needed. 

VIDEO6.First Renewal of Bid #18/19-28, EMS Medical Supplies (C. Leroy/D. Motes)
1) Approval of the First Renewal of Bid #18/19-28 EMS Medical Supplies to multiple vendors based on a per item cost and availability as outlined on the attached spreadsheet. 2) Additional approval to continue the waiver of the Purchasing Policy as it may apply and when necessary, for purchase of medication, supplies, including janitorial and cleaning supplies. This request is due to shortages, price fluctuation and packaging of these items. Additional request to extend this waiver to the Animal Services Division for purchase of these same items.  See attached memos for additional information. VIDEO8.Supplemental Agreement #2 to LAP Agreement #2018/2019-58 with the Florida Department of Transportation and the Accompanying Authorization Resolution for Sidewalks & Pedestrian Bridges along Pine Avenue from Old Fleming Grove Road to Raggedy Point Road (R. Smith)

1) Approval of Supplemental Agreement #2 to Agreement #2018/2019-58 with the Florida Department of Transportation, regarding LAP #G1546, Sidewalks and Pedestrian Bridges on Pine Avenue (from Old Fleming Grove to Raggedy Point Road), extending the project term until 10/31/2022. All other terms and conditions remain the same.
