Approval of the Adult Drug Court FY22/23 Funding Agreement with Clay Behavioral Health Center, Inc. for mental health and substance abuse treatment services in the amount not exceed the total sum of $14,008.33 per month/$168,100.00 per fiscal year.  The term of the Agreement is 10/1/22 through 9/30/23, pending FY22/23 Budget approval.

Approval of the Ninth Renewal and Modification to Agreement No. 2000/2001-164 with Scibal Associates, Inc. d/b/a Qual-Lynx, regarding claims administration services (Workers' Compensation), for the term of 10/1/22 through 9/30/23, at the pre-negotiated rates listed in Exhibit B, with all other terms and conditions to remain in full force and effect.

Review and approval of SHIP Annual Report and Local Housing Incentives Certification to be submitted to Florida Housing Financial Corporation. The report includes Closeout for FY19/20 and Interim FY20/21.

Approval of the FY22/23 Memorandum of Agreement (MOA) with the Clay County Sheriff's Office, for security services at the Clay County Administration Building, for the term of 10/1/2022 through 9/30/2023, in the amount of $66,981.40.

Approval of Agreement with D.E. Scorpio Corporation d/b/a Scorpio for Phase I Preconstruction for Construction Management at Risk (CMAR) Services for the Fleming Island Health Department Facility Renovation.  Phase I total is $45,000.00.

Approval of Second Renewal and advance payment to Agreement No. 2019/2020-206, Vector Solutions Learning LLC dba Vector Solutions in the amount of $40,587.84.  Term of the Agreement is 10/1/22 through 9/30/23.

Approval of the Commercial Lease Agreement with Big Score Investors, LLC, c/o Colliers International Northeast Florida for the premises located at 400 College Drive, Suite 106.  Monthly rental fees are stipulated within the Agreement.  Lease term begins on October 1, 2022 through November 30, 2027. Premises will be used by the Clay County Paramedicine Group team.

Approval of the Public Emergency Medical Transportation Letter of Agreement (LOA) with the State of Florida Agency for Health Care Administration, regarding the remittance of Intergovernmental Transfers (IGTs) funds in an amount not to exceed $398,256.49. 

Approval of the Emergency Management Performance Grant (EMPG) Sub-award Grant Agreement No. G0420 with the State of Florida, Division of Emergency Management, in the not to exceed amount of $73,632.00.  The agreement term begins 10/1/22 through 09/30/23.  The County is required to match dollar-for-dollar the total award amount.

Approval to submit required certifications to accompany the State Aid to Libraries Grant Application and execute related grant Agreement to support general operating expenses for the Clay County Library System.

Approval of the Second Amendment to the Fairgrounds Lease Agreement with Clay County Fair Association Agreement No. 2018/2019-79-AM2.  This Amendment modifies the First Amendment which addressed a Capital Project known as the Restroom Facility Project and acknowledges that no credit is due for this project, all standard rental fee s will apply.
