Approval of Agreement with Playcore Wisconsin dba GameTime in the amount of $262,771.97 for new equipment at W.E. Varnes Park and Island Forest Park. Each project is to be completed in 120 days from Notice to Proceed.

Approval of the Agreement for Professional Lobbying Services with The Southern Group of Florida, Inc. in the amount of $6,000.00 per month ($72,000.00 annually) for a term of three (3) years from the effective date.

VIDEO3.Advance Payment for the purchase of a Pumper and Ladder Truck for the Public Safety Department (J. Boree)

Approval of advance payment for the purchase one (1) Pumper Truck and one (1) Ladder Truck from Ten-8 utilizing Sourcewell Contract No. 113021 OKC-1 pricing. Ten-8 will provide a pre-payment discount. A performance and payment bond will be required from Ten-8 prior to advance payment.

VIDEO4.Supplemental Agreement with AT&T for Emergency Services IP Network (ESInet) – Next Gen 911 Solution Managed Services (D. Hane / D. Barro)

Approval of a Supplemental Agreement with AT&T for Emergency Services IP Network (ESInet) – Next Gen 911 Solution Managed Services. Total recurring monthly charges will be $26,498.98 for 60 months beginning on the effective date with the option to extend the service for an additional five (5) one-year terms.

Approval of the Seventh Amendment to Agreement No. 2010/2011-57 with Blue Cross and Blue Shield of Florida, Inc. for Administrative Services. 

This amendment extends the term until 12/31/2025, amendments financial arrangements, and amendments pharmacy arrangements. 

 

VIDEO6.First Amendment to Agreement No. 2021/2022-208 with the State of Florida Department of Environmental Protection regarding the Flooding Vulnerability Assessment Grant (M. Mosley)

Approval of the First Amendment to Agreement No. 2021/2022-208 with the State of Florida Department of Environmental Protection regarding the Clay County Flooding Vulnerability Assessment Grant. The Department has amended as outlined in Attachments 1, 2, and 3.

The TDC has recommended a grant award amount of $4,000.00.
