disposal of various Board property and various documents are submitted
for the Board's information. At times, approval is requested for various
finance related issues.
7.Approval of Interlocal Agreements with Municipalities for the Collection of Comprehensive Impact Fees adopted by Ordinance No. 2022-56 (C. Grimm)

a. Interlocal Agreement between Clay County and the Town of Orange Park

b. Interlocal Agreement between Clay County and the City of Green Cove Springs

c. Interlocal Agreement between Clay County and the City of Keystone Heights

d. Interlocal Agreement between Clay County and the Town of Penney Farms

8.Grant of Conservation Easement to SJRWMD in connection with Regional Sports Park (C. Grimm)
9.Agreement for Light Pole Conversion and LED Upgrade at Armstrong Park with Musco Sports Lighting, LLC. (J. Pierce)

Approval of the Agreement for Light Pole Conversion and LED Upgrade at Armstrong Park with Musco Sports Lighting, LLC in the amount of $280,320.00.  The project shall be completed within 120 calendar days.

10.Second Amendment to Agreement No. 2020/2021-207 with Agrow Pro, Inc. for Agronomic Services (J. Pierce)

Approval of the Second Amendment to Agreement No. 2020/2021-207 with Agrow Pro, Inc. for Agronomic Services for a total annual cost of $182,372.45.  This Amendment extends the services for one additional year, adds additional services, and modifies the type of applications utilized.

11.First Extension to Agreement No. 2022/2023-49 with Bryan's Landscaping Services, LLC for Mowing Services (J. Pierce)

Approval of the First Extension to Agreement No. 2022/2023-49 with Bryan's Landscaping Services, LLC for Mowing Services through May 31, 2023 or until such time as a new contract has been executed.

12.Acceptance for Final Plat for Recording Shadow Crest at Rolling Hills (District 5, Comm. Burke)(R.Smith)
The applicant is requesting that the Board accept for recording the final plat of Shadow Crest at Rolling Hills. The plat consist of 247 single family lots and is located off of Sandridge Road.
13.RFQ No. 22/23-13, Tourism Advertising Agency (K. Smith)

Approval to accept staff's ranking and selection of the top ranked Firm for RFQ No. 22/23-13,Tourism Advertising Agency. Approval of award will be effective after the 72 hour protest period has expired. 

 

1.Advance Travel and Tourism - 94.67

3.Daigle Creative - 59.67

 

A committee consisting of Director of Communications, Director of Tourism and Film Development, and Director of Parks and Recreation performed the evaluation and rank of the responses received. If desired, the Board may request presentations from the firms. Agreements will be negotiated and brought back to the Board for approval.

A.) Approval of the Grant Agreement for the E911 Rural and State Grant Program with the Florida Department of Management Services, S22-23-01-11, regarding the Region 3 Repository Project, total reimbursable amount not to exceed $303,796.00.

A.) Approval of the Grant Agreement for the E911 Rural and State Grant Program with the Florida Department of Management Services, S22-23-01-12, regarding the Logging Recorder Project, total reimbursable amount $84,492.50.

 

B.)  Approval of the accompanying Budget Resolution.

Agreement with D.E. Scorpio Corporation dba Scorpio for Phase 2 Agreement for the Florida Department of Health, Fleming Island buildings 400 and 600 construction and renovation project at the Guaranteed Maximum Price of $5,712,470.00. Final construction completion time is 410 calendar days. 
17.Service Agreement between Clay County and Flagler Hospital regarding Care Connect Information Network (CCIN) (V. Hapner)

A.) Approval of the Service Agreement with Flagler Hospital regarding Care Connect Information Network (CCIN) in the amount of $100,000.00 annually for a term commencing on the effective date for a period of two (2) years.

B.) Additional approval of Sole Source.

C.) Additional approval of semi-annual advanced payment of $50,000.00 ($100,000.00 annually)

This application is a Small-Scale Comprehensive Plan Amendment to the 2040 Future Land Use Map (FLUM). The application would change the Future Land Use (FLU) designation of two (2) parcels of land from Urban Core-10 to Urban Core-16.

 

B.) PUD-2022-11

This application is a Rezoning of two (2) parcels from AR (Agricultural Residential) to PUD (Planned Unit Development).

 

The applicant has requested this item be Continued to the March 28th BCC meeting to allow them time to respond to input from meetings held with the local community.
