to meet the requirements of Ch. 136.06 (1) FL Statute. Acceptance and
disposal of various Board property and various documents are submitted
for the Board's information. At times, approval is requested for various
finance related issues.

Approval of the Subrecipient Agreement for American Rescue Plan Act (ARPA) SLFRF Funds with Clay County Development Authority (CCDA) pending receipt the Unique Entity ID (UEI) number through the System for Award Management (SAM.gov).  This is a reimbursable sub-recipient grant and will not exceed $200,000.00.

1. Blue Angels Kiosk (Product Development Grant)- Recommended Award $10,000.00 (8-0 vote).

2. Moosehaven Chili Cookoff (Signature Event Marketing Grant)- Recommended Award $5,000.00 (8-0 vote).

3. Moosehaven American Pride 4th of July (Special Event Marketing Grant)- Recommended Award $3,500.00 (8-0 vote).

4. Hometown Holiday (Special Event Grant)- Recommended Award $3,500.00 (8-0 vote).

5. American Cornhole League (Sports Event Grant)- Recommended Award $7,500.00 (8-0 vote).

6. Cattle at the Cove (Sports Event Grant)- Recommended Award $5,000.00 (8-0 vote).

9.BlueMedicare Group Master Agreement with Blue Cross and Blue Shield of Florida Inc. for 2023 (J. Leighton)

A) Approval of BlueMedicare Group Master Agreement with Blue Cross and Blue Shield of Florida, Inc. (Florida Blue), regarding the group Medicare Advantage, Medicare Advantage Prescription Drug Plan, and/or stand-alone Medicare Prescription Drug Plan for retirees and covered dependents, effective 1/1/2024.

B) Additional approval of advance payment for the first premium payment.

10.Eighth Amendment to Administrative Services Agreement No. 2010/2011-57 with Blue Cross and Blue Shield of Florida, Inc. d/b/a Florida Blue (J. Leighton/T. Nagle)

Approval of the Eighth Amendment to Administrative Services Agreement No. 2010/2011-57 with Blue Cross and Blue Shield of Florida, Inc. d/b/a Florida Blue revising and replacing Exhibit B, Financial Arrangements.  All other terms and conditions remain unchanged and in effect through December 31, 2025.

11.Bid No. 22/23-86 Green Cove Springs Augusta Savage Arts & Community Center Pickleball Courts Project (K. Smith)

Approval to reject bid received from Southeast Paving, Inc. for Bid No. 22/23-86, Green Cove Springs Augusta Savage Arts & Community Center Pickleball Courts Project. The one bid received is over budget. Staff will immediately re-advertise the bid in an attempt to received more competitive bid submittals. 

12.Approval of Grant of Permanent Easement for Drainage between Clay County and Autumn Underwood (Parcel 803) and between Clay County and Amber Underwood (Parcel 804) (C. Grimm/E. Dendor)
13.Third Amendment to Agreement No. 2020/2021-150 with Hubbard Construction Company

Approval of the Third Amendment to Agreement No. 2020/2021-150 for Continuous Asphalt Resurfacing, Paving and Supplies with Hubbard Construction Company, amending the terms and eight of the unit prices due to unforeseeable increases in costs.  This Agreement is utilized for various projects as needed.

14.Supplemental Agreement with Florida Department of Transportation regarding CR 218 from Clay Hill Elementary to Taylor Road Project (K. Smith)

Approval of the Supplemental Agreement with Florida Department of Transportation (FDOT) regarding the construction of a sidewalk on CR 218 from Clay Hill Elementary to Taylor Road.  FDOT has committed to contribute an additional $212,563.40 with the County contributing $100,000 which will allow the County to move forward with awarding the construction bid.

15.Bid No. 22/23-32 CR 218 From Clay Hill Elementary to Taylor Road Sidewalk Improvement Project (R. Smith)

Approval to post the Notice of Intent to award Bid No. 22/23-32, CR 218 From Clay Hill Elementary to Taylor Road Sidewalk Improvement Project to CDM Contracting, Inc. in the amount of $889,499.40.  Approval of award will be effective after the 72-hour protest period has expired.  All work will be completed within One Hundred and Thirty (130) days from Notice to Proceed. Majority of this project is being funded by FDOT, with the County providing some additional funds necessary to complete the project.
